Designated Passport Acceptance Facilities

The Office of the City Clerk offers two locations for processing passport applications:

Municipal Archives & Records Facility is ADA accessible, provides free parking, and will process photos. As an added benefit, this specific location is conveniently located next to the Vital Statistics Office should individuals require a certified copy of their long form birth certificate.
The City Hall location is ADA accessible and will process photos. Free parking is limited.

The following items are required when you apply for a passport:
     1. Completed form DS-11(Passport Application)
     2. Evidence of U.S Citizenship (see list of valid options below)
     3. Primary Form of Identification (see list of valid options below)
     4. 1 Passport Photo
     5. Application Processing Fees (refer to Passport Processing Fees below)
