Join the annual effort and check your numbers in 2011, and be entered into a drawing for a chance to win a Wii Fit!
The City of San Antonio understands the importance of your health and is pleased to present you with the opportunity to increase your health awareness by “Knowing Your Numbers.” The City will offer health screenings at various locations on dates from
March 22 through April 7 for active, full-time, civilian employees.
The health screenings are free for all active, full-time, civilian employees and you will receive one (1) hour during the workday to participate. Register today at www.provantevents.com/CityofSanAntonio and coordinate with your supervisor to attend. You do not have to be on the City’s health plan to participate.
Health screenings include:
- Non-fasting finger stick
- Cholesterol
- Glucose
- Blood pressure
- Height, weight, BMI, body fat
- 10-minute consultation with a UnitedHealthcare Coach
Registration is easy! If you are on the City's health plan, you will need a copy of your UnitedHealthcare ID card to register.
1. Visit www.provantevents.com/CityofSanAntonio.
2. Enter your e-mail address in the required field.
3. Click on "No, I am a new user."
4. Enter your information to create your personal profile (if you are not on the City's health plan, enter 0 in the spaces for Member ID and Group ID).
5. Click "Signup."
6. Click on the "All Events" option from the navigation bar.
7. Select your location and time.
8. Click "Register." You will receive an e-mail reminder.
Remember, attending four (4) Employee Wellness Program events is an eligible activity for the
2011 Wellness Rewards Program, and a Know Your Numbers event can count as one of the events!
